If you are looking to schedule a therapy appointment or take a tour please call our office at any of our locations to schedule your appointment. Please indicate what you will be seen for and if you need Physical Therapy, Occupational Therapy, Speech Therapy or Aquatic Therapy. You can request to have the intake paperwork emailed to you to get most of it completed early. However, we still encourage you to arrive 15 min prior to our initial appointment. Please wear comfortable clothing that will allow the therapist access to the affected area. Your initial appointment will be approximately 45 min to 1 hour. We have appointment times for initial patients as early as 7am and as late as 4pm.
For your Initial appointment: Please bring your Driver’s License or photo ID, Insurance Cards, and your therapy prescription from your practitioner. If requested, make sure to go to the website in your initial email to complete most of the patient intake forms. If you don’t want to do it online, its ok, just arrive 15 min early and we can complete it on paper.

CASH BASED OPTIONS: We also offer reasonable, cash based options for appointments if patients do not have insurance coverage or if they have a plan with high deductible. Cash based options can also be used if the patient wishes to not involve their insurance or if the patient desires a service not covered by their insurance plan, please call our clinic for rates.
Payment Plans: We offer Care Credit as an option to help finance the cost of therapy. We also have in-house no finance payment plans.

Good Faith Estimate
You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least one (1) business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate.
